Welcome

• We strive to provide you and your family with the best possible patient experience. Please arrive to the office 15 minutes before your scheduled appointment time so  our staff can meet you and properly gather all necessary information including a copy of your government issued photo identification (State ID/Driver’s License/Passport) and your current insurance card(s). In advance, you may print out and complete the new patient packet located in the Patient Forms section of this website. Patients under the age of 18 must be accompanied by a parent or legal guardian.  
• Due to allergies of patients and staff-we request you limit fragrances (perfumes, colognes, deodorant, hair spray etc).

THERE IS A $75.00  "NO SHOW" OR LESS THAN 24 HOUR APPOINTMENT CANCELLATION FEE

If the office is closed and you need to cancel your appointment you can send us an email notification to: info@eyecarecenterlakecounty.com to avoid a late notification fee or no show fee if received with more than 24 hours advanced notification.
