There is a $25.00 fee for "No Show" or appointments cancelled with than 24 hours notice.
You can now pay your bill with InstaMed without having to log into Nextgen (Medfusion) patient portal.
Every effort is made to assure that your correct insurance(s) is billed using the information that you have provided to the staff on the date of service. While you may have both a medical insurance plan and a vision insurance plan, we cannot bill both on the same date of service. You will need to indicate to the front desk at the time of check-in which plan you would like billed for services rendered. There is a form that the front desk will give you at the time of check in to indicate the insurance you are requesting us to bill. Please make sure we have accurate and current insurance information.
If you later decide you want a different insurance to be billed other than what was provided or elected at the time of service, you will need to submit the claim yourself.
Note as of 8-1-2022 we are not longer participating in LCPA-Humana Plans
We participate with most major insurance companies. However, it is very important that you understand your specific insurance policy and adhere to any requirements such as obtaining a referral prior to your appointment. Please call your insurance company or if you are an HMO patient call your primary care doctor for any questions about your policy.
